Requirements need to submit for normal delivery.

The requirements are as follows:

1. Duly accomplished Maternity Notification Form and Allocation of Maternity Leave Credits Form
2. Ultrasound report
3. Photocopy of SSS ID or 2 valid IDs
4. Certified true copy of birth certificate of child from local civil registry office or PSA copy of birth certificate
