How to file SSS Sickness Notification.

By submitting the following requirements:

1. Duly accomplished SSS sickness notification form that can be downloaded from https://www.sss.gov.ph/sss/DownloadContent?fileName=SIC_01252.pdf

2. Medical certificate

3. Photocopy of SSS ID or UMID ID or 2 valid IDs

4. Certified true copy of operative record (if operated)

5. Certified true copy of clinical abstract (if confined)

6. Certified true copy of discharge summary (if confined)

 

Scanned copy of the requirements will do.
