How to replace lost SSS ID.

Visit any SSS office and submit the following:

1. Accomplished UMID Card Application Form
2. Accomplished R-6 Miscellaneous Payment Form
3. Affidavit of Loss (AOL)
4. Original and photocopy of valid IDs
